How to create a form: style, logic, assessment, and submissions
Open Forms in the left sidebar. This is the studio. If someone turns the studio off, shipped forms can still collect answers. Each card on the grid shows name, status, whether it is public, and the style if it is not Default. Filter by status. Search by name.
How to create a form
- Click the New Form card, or the green + in the header.
- Describe the form to the assistant, or set it up yourself. Same idea as adding a page in Config.
- Add fields in order. Example: Name, Email, “How can we help?”, and a file upload.
- Set who can see it (public link vs signed-in only).
- Save and copy the share link. Embed code is on the same settings if you want the form on another site.
Example: a “Talk to sales” form with name, work email, and company. A visitor submits. You read it under Submissions.
How to pick a style
Open the form. Open Design.
- Style: Default (one page, usual fields), Spacious (more air), Minimal (tighter), or Conversational (one question or group at a time, a welcome screen instead of a header).
- If it is not Conversational, Labels: Vertical, Minimal, or Horizontal (wide label).
- If it is Conversational, you can skip the welcome screen and open on the first group or question.
- Font size: a few presets. Colors: Public colors opens the brand palette used on forms and other public pages. See Brand kit.
The card on the grid tags the style when it is not Default.
How to add logic
Open the form. Open Logic. Add fields first. Then Add Rule.
- IF a field matches (is equal to, contains, is empty, and so on).
- THEN show a field, hide a field, skip to a field, redirect to a URL, or show a message.
Example: if “How can we help?” is “Support”, show a ticket-style question. If it is “Sales”, skip to company size.
How to turn on scoring and assessment
This tab appears when the form has steps (groups of questions). Open Scoring.
- Set how scores roll up at the group and at the step (sum, average, and the options on those cards).
- Show live score if you want a progress panel while someone fills it. Show score chart if you want a radar, bar, or stacked view while they fill.
- Turn on Assessment Results to show a personalized result after submit. Add bands per step: score range, title, and action items the person should see.
- Optional: after they view the result, redirect to a URL.
- Optional: on approval, create a record for each matched action item and map fields from the assessment item. The parent assessment row is created first.
Example: a readiness form with three steps. A low band on “Go to market” shows “Write a one-page offer” as an action item. The visitor sees that card on the success screen.
How to set captcha, languages, CRM, and tickets
- Open the form. Turn on captcha if public spam is a risk. A visitor must pass it before submit.
- Add languages if you want the same form in more than one language. The visitor picks or gets the matching locale.
- Write the success message they see after submit (unless an assessment result replaces that screen).
- Turn on CRM connect so a submit creates a contact under a company. See CRM.
- A form can also open a ticket when a workflow or the form target is set that way. See Projects.
How to read submissions
- Open the Submissions tab.
- Choose a form on the left. That form’s inbox loads.
- Open a row to read the answers. Use this to follow up. Some workspaces can mark a row processed or approved from here.
You do not add submissions here. Visitors do. You do not invent history.
Sparkles or Ctrl/Cmd+J opens the Forms assistant. Full steps: page assistant.
